
Call Auntie Community Health Workers are a special bunch of humans dedicated to supporting you through more challenging life moments.
From gentle appointment reminders and accompaniment, to helping you make those difficult phone calls, navigating the housing system and other community resources, Community Health Workers can be a life admin helper during the times when you feel overwhelmed and under supported. Our team supports families through pregnancy, birth and the postpartum period, including advocacy with child aid societies. We meet folks where they are at and support Indigenous families to stay together and self- determine what is best for themselves and their family.
We are happy to work with your trusted team of support agencies and health care providers! Community Health Workers collaborate with the Call Auntie Street Outreach and Clinical Team to connect community members to dignified care.
The cycle of the Community Health Work Program is every 3 months. This means your Community Health Worker will have a discussion with you at the 3 month point to make a plan for transitioning out of care or to another community agency or make a plan to continue on in the Community Health Work Program. Every person and family is different and we honour this, so some relationships may be longer than others.
